Report form for notification of occurrence involving dangerous goods shall be used to report all accidents, incidents and near-misses related to transport of dangerous goods by road or rail, when the occurrence had or could have consequences for life, health, environment or material values. The obligation to report includes occurrences during loading, unloading and carriage of dangerous goods, and the report must be completed within 8 days after the occurrence. Incidents during transport of dangerous goods causing injury to persons shall always be reported, even if the dangerous goods is not involved in, or the cause of, the injury.

The involved participants (see ADR or RID chapter 1.4), has the duty to report. The participant may decide themselves who shall complete the form. Anonymous reporting is possible.
The objective of registering occurrences involving dangerous goods is to monitor incidents in order to prevent accidents and to enhance knowledge of correct dimensioning, planning and training in preparedness in relation to hazardous substances.
