When should you use this form?

Employers are obliged to notify the police and the Labour Inspection Authority of accidents at work in which an employee is fataly or seriously injured. This form applies to the notification of such accidents at work to the Labour Inspection Authority.

This reporting obligation is stipulated in Section 5-2 of the Working Environment Act (arbeidsmiljøloven).

Who should use this form?

The obligation to report to the Labour Inspection Authority applies to the employer of the employee who has been fataly or seriously injured.
