Notification of changes for an authorised occupational health service
All authorised occupational health services are obliged to notify the Norwegian Labour Inspection Authority of changes that affect their authorisation.
In addition, it is desirable that the company also reports changes that can help to keep the Labour Inspection Authority's register up to date. For example, contact information, departmental information, telephone numbers, addresses or changes in the professional set-up of the organisation.

Who should use this form?
All authorised occupational health services are obliged to notify the Norwegian Labour Inspection Authority of any changes that affect their authorisation.
