Use this form to submit the mandatory summary reports set forward in the grant letter for aid to short sea shipping.
The summary report must include information related to the total amount of transferred freight volumes, the freight's origin and destination points, as well as a financial summary report. An account for any contingent deviation from the business plan should also be included if needed.
The specific report deadlines are specified in the grant letter.
Stipulations regarding the submission of summary reports are specified in the grant letter.
In order to complete and submit this form you may use one of the following options:
• Log in with your national identity number or a D-number
• Sign in with your enterprise certificate. Note that the appropiate role must be assigned to you.

Apart from a financial summary report for the relevant period, it is mandatory to include an independent auditor's certificate.
You may also wish to include other documents, i.e. bills of lading.
The grant payment depends on the submission of the report.
