The form should be used when applying for permit the first time or when renewing an expired permit.
Only commercial air operators carrying cargo or passengers (CAT operators) are required to fill in the form.
In addition to filling in the form, attachments documenting how the requirements are met needs to be uploaded.
Publicly categorised airports have conditions attached to their technical/operational certificate that requires them to ensure commercial operators are made aware of the airport’s particular operational requirements, and that they can document compliance with these requirements.
The form is divided into an administrative part containing contact information and operator license. Next section lists airport(s) and aircraft type(s) that will be used, and lastly the individual airport requirements where files containing compliance documentation must be uploaded. The form will not submit unless all mandatory fields are filled out.
